1010 Form Information

1.  Send an email to parkern@hcboe.net

2.  In subject line:  Requesting 1010 Form.

3.  Within the body of the message include:  your full legal name and birthday.

*You will receive an email verifying if/when your form is processed and if you will need to pay.

(Reminder:  1st form for the school year is free, all others will cost $3.00)

Forms are good for 30 days from the day they are processed.

FORMS MUST BE REQUESTED 3 DAYS IN ADVANCE!
